Welcome to our office! Please remember to bring your completed forms including a list of any supplements and/or medications you are currently taking along to your first appointment.
Please familiarize yourself with our policies and fee schedule as outlined below so we can best focus our attention on your health care needs when you are here.
Your appointment time is set aside especially for you and your practitioner. Our office requires 24 hour notice in the event that you must cancel your appointment. Patients who miss their appointment or cancel at the last minute will be charged for the full amount of the appointment. Please understand that our practitioners dedicate a significant amount of time for each patient visit, and this time lost represents a person who could have received treatment. Our office employs an automated reminder system that can contact you by phone, text message or email 48 hours before your scheduled appointment—if interested, please request this when scheduling.
We appreciate a call if you are going to be more than 10 minutes late for an appointment. Unfortunately, we cannot guarantee that "latecomers" will receive full time for their treatment.
Out of courtesy to other patients, please keep noise levels down while in the office.
A full pharmacy of Chinese & Western herbal medicines and an inventory of nutritional supplements are stocked in our office. Whenever possible, plan to pick these up during your office visit and let your practitioner know what you need prior to your treatment.
Between visits, phone and email consultations are available. In the case of consultations requiring more than a few minutes, insurance may be billed.
We may request a credit card number to reserve your initial appointment.
Payment is required at the time that services are rendered unless prior arrangements have been made. This includes insurance patients whose coverage must be verified prior to billing or validated after initial submission.
Our office is equipped to bill insurance companies directly and we are happy to offer this service to our patients. Please note that your insurance policy is an agreement between you and your insurance company, and that you are personally responsible for all of the charges to your account should your insurance company refuse payment for services rendered. If after 90 days, we have not received full payment from your insurance company, your account will be billed any outstanding balance. All deductibles, co-payments and `patient portions' are due at time of service.
Please bring your insurance card to your initial visit or fax a copy of the front and back sides to the office beforehand to expedite the processing of your account.
To cover our administrative costs, we will add a late charge of a minimum of $10 or 1.5% (whichever is greater) on all accounts not paid in full each month. If your check is returned from the bank, we will add a "returned check" fee to your account in the amount of $20.
Insurance will be billed for all modalities employed in your session with the exception of herbs and supplements which are not covered. In the event that you are uninsured or have poor coverage you may request to sign a hardship waiver where you would be billed directly at the lowest possible rate for initial consultation and acupuncture only, with all other modalities waived.
Initial evaluation, consultation and treatment can be from 60-90 minutes. The hardship rate for an initial visit is $170.
Follow up treatments last approximately 45-60 minutes, though if a case warrants extra time needed for consultation and/or treatment your practitioner may recommend a longer follow up session. For a normal length follow up the hardship rate is $85.
