Notice of Privacy Practices: Our Clinic Protects Your Health Information and Privacy

Dear Valued Client,

This notice describes our office's policy for how health information about you may be used and disclosed, how you can get access to this information, and how your privacy is being protected.

In order to maintain the level of service that you expect from our office, we may need to share limited personal health and financial information with your insurance company, or with other health practitioners that you authorize.

It is the policy at Artemis Healing Center to maintain confidentiality of all client information. Although the practitioners at Artemis all work within the same office, each maintains a private practice separate and apart from one another. Each individual practitioner maintains all client files separate and apart from one another and your health information shall not be shared with anyone at or outside of Artemis without your consent and agreement.

No practitioner at Artemis is privy to what the other practitioners are doing with respect to their clients, whom they see or consult with, or what regimen or procedures they may prescribe. Each practitioner maintains her practice in the manner she chooses, and has no ability to influence the methods or manner of any other practitioner.

Your information may be used and disclosed, with your authorization: for session, payment, and health care operations; to family involved in your care; to remind you of appointments, or to give you information about session alternatives or health-related services; workers' compensation and insurance claims; as permitted or required by law.

Safeguards in place at our office include:

  • Limited access to facilities where information is stored.
  • Policies and procedures for handling information.
  • Requirements for third parties to contractually comply with privacy laws.
  • All health files and records are kept on permanent file.

Types of information that we gather and use:

In administering your health care, we gather and maintain information that may include non-public personal information:

  • About your financial transactions with us (billing transactions).
  • From your health history, session notes, all test results, and any letters, faxes, emails or telephone conversations to or from other health care practitioners.
  • From health care providers, insurance companies, workman's comp and your employer, and other third party administrators (e.g. requests for health records, claim payment information).
